rescan: say when a skill exists but is not offered

the button counts skills — one directory with a SKILL.md is one skill. it now also reports how many are actually OFFERED to an agent, which is a different number: the loader skips any without a `description`, because a line that does not say when to reach for something costs context in every conversation and earns nothing. so a skill can be present, listed, editable in the UI, and invisible to every agent, with nothing anywhere saying why. the toast names the gap and the reason rather than leaving two numbers to be noticed by whoever thinks to compare them.
